420.1 Rule 

School District of Prairie Farm

 

Any child who enters or re-enters the Prairie Farm School District from a non-public school or home-based private educational program shall be required to comply with the following procedures:

 

A. Registration

It shall be the responsibility of each parent or guardian of a child or the child if of legal age, entering or re-entering the Prairie Farm School District to complete official registration forms in the District Administrator’s office prior to the assignment by the Pre-K12 principal to a grade or a schedule of classes.

 

B. Transcripts

Students transferring from other school systems or home-based educational programs are required to provide a transcript or other record of academic accomplishments and other available records, including attendance records, level of academic achievement, subjects completed, credits earned, and records of standardized achievement testing. As an alternative, school officials may elect to have parent, guardian and/or student sign a release form to allow the Prairie Farm School District to obtain such information. Parents of children from home-based programs are required to meet with the Pre-K12 principal and/or staff  members to review materials used in home-based  programs to determine levels at which the home-based children were instructed.

 

C Health Records and Physical

Parents or guardians of students admitted to the District Elementary and Secondary Schools shall present immunization records as required by law. In addition, students are encouraged to have complete physical and dental examinations prior to entering or re-entering the school system.

 

D. Placement

 

Students who desire to be admitted to the Prairie Farm School District from a home-based private educational program shall be assessed to determine appropriate educational placement.

 

The District may require that a placement assessment be completed so that the student shall be placed at the appropriate grade level. The evaluation process may also consist of:

            1. Achievement tests

            2. Diagnostic tests

            3. Criterion-referenced tests

            4. Mental abilities test

            5. Parent checklist

            6. Counselor interview

 

In making the placement decision under the above provisions, the District shall also consider the age, mental age, social and emotional development and academic progress of the student. Placement shall be discussed with an adult student, parent or legal guardian before becoming effective.

 

E. Appeals

A parent, legal guardian or adult student may appeal a student placement in writing to the District Administrator. The placement may also be appealed to the Board. The placement decision made by the Board shall be considered as final.

 

F. Probationary Period

Students placed under this policy shall be subject to a probationary period of thirty school days to determine whether original placement was in the best interests of the student. Student progress shall be monitored carefully during the probationary period.


 

G. Graduation Status

In order for a home-based student to graduate with a Prairie Farm High School diploma, he/she must satisfy the District’s graduation requirements.
